Medicare Plans
Part D Drug Plans in San Antonio, TX
Part D is Medicare's prescription drug benefit, sold through private insurers. The plan with the lowest sticker premium is often the priciest one once you factor in the specific drugs you take.
The only reliable way to shop Part D drug plans in San Antonio is to run your exact prescriptions, dose, and pharmacy through each plan's formulary. That's the exercise we walk through with clients across Bexar County every fall.
- Priced against your actual medication list, not the sticker premium
- Which pharmacy you use can change your copay
- Formularies reset every January 1
- Extra Help can lower costs if your income qualifies

Get your prescriptions priced for the full year, not just the premium
Send us your medication list and the pharmacy you use, whether that's an H-E-B pharmacy near Stone Oak or one downtown. We price the full year, premium, deductible, and expected copays, across the plans we represent and call you with the numbers.

The basics
Four factors that decide your actual cost
The formulary
Each plan covers a set list of drugs. If yours isn't listed, you're stuck paying full retail or requesting an exception. The tier
Covered drugs land on tiers ranging from preferred generic to specialty. The same drug can sit on a cheap tier in one plan and an expensive one in another. The pharmacy
Preferred pharmacies, including many H-E-B pharmacy locations, usually cost less than standard ones for the same fill.
Shopping
How we shop Part D drug plans in San Antonio
- 1
Write down every prescription
Include the dose and how often you refill it. Bring the bottles or a pharmacy printout, since guessing is where mistakes creep in.
- 2
Tell us your pharmacy
Whether you fill near the Medical Center District, at an H-E-B pharmacy in Stone Oak, or through mail order, preferred status changes the price.
- 3
Compare the full year, not the premium
Premium plus deductible plus every expected copay across twelve months, never the monthly price alone.
- 4
Recheck it every fall
Formularies and tiers reset on January 1. A plan that worked well last year can turn into the wrong one this year.
Watch out
Penalties, coverage gaps, and help paying
Go 63 days or longer without creditable drug coverage after you first become eligible, and Medicare tacks a late enrollment penalty onto your Part D premium for good. It looks small monthly, but it adds up over a retirement, and it's one of the mistakes we see most often among new enrollees in San Antonio.
Lower-income residents in and around San Antonio may qualify for Extra Help, which reduces or wipes out Part D premiums and deductibles. See our San Antonio senior resources page for where to apply.
Coverage through an employer, TRICARE, or the VA may already count
Retirees connected to Joint Base San Antonio who carry TRICARE for Life, along with VA or employer coverage, usually avoid the penalty automatically. Hold onto the yearly creditable-coverage letter your plan mails you.
Most Advantage plans already bundle Part D
If you're on a Medicare Advantage plan that includes drug coverage, adding a standalone Part D plan can actually drop you out of your Advantage plan.
- Enroll as soon as you're eligible to avoid a permanent penalty
- Standalone Part D pairs with Medigap, not with most Advantage plans
- Ask your doctor about generic alternatives before accepting a specialty tier
- Review your coverage every Annual Open Enrollment period
Cost stages
Deductible, copays, and the annual out-of-pocket cap
| Stage | What happens | What to watch |
|---|---|---|
| Deductible | Some plans apply a deductible before cost sharing kicks in; others waive it on lower tiers. | A $0-deductible plan isn't automatically the cheaper pick, check the tier copays behind it. |
| Initial coverage | You pay a copay or coinsurance for each fill while the plan covers the rest. | Tier placement drives this stage. The same drug can sit on tier 2 in one plan and tier 4 in another. |
| Catastrophic / annual cap | Once your out-of-pocket drug spending hits the yearly limit, covered drugs cost nothing for the rest of the year. | This cap matters most if you're on a specialty or brand-only medication. |
The Medicare Prescription Payment Plan
Medicare now lets you spread out-of-pocket drug costs across monthly installments instead of paying a large amount at the counter early in the year. It doesn't lower your total cost, it just spreads it out. Worth asking about if a January refill has ever hit your budget hard. Sign-up runs through your drug plan, and we can walk you through it.
Every fall
Your yearly medication checkup
Between October 15 and December 7, every Part D drug plan in San Antonio can change its premium, deductible, formulary, tier placement, and preferred pharmacy network. Your own prescriptions can shift too. A plan that was your cheapest option last January can quietly turn into one of the most expensive by fall.
Each fall we run your current medication list against the plans we represent and tell you whether staying put or switching saves more over the full year. If staying is the right call, we'll say so directly. See Annual Open Enrollment for the exact dates.
