Getting started
Turning 65 San Antonio: Your Medicare Timeline
Turning 65 in San Antonio starts a seven-month countdown. That window opens three months before your birthday, includes your birthday month, and stays open three months after, and it's when you lock in Part A and Part B.
Apply during the three months before your birthday and your coverage kicks in on the first of your birthday month. Wait past that and your start date slips. That's why we tell clients from Stone Oak to Southtown to call us at 64 and a half, not 65.
- A 7-month window built around your birthday
- Different rules if you're still on the job
- One shot at guaranteed-issue Medigap
- Free, local guidance from a Bexar County agent

Timeline
Turning 65 San Antonio, month by month
- 1
3 to 6 months before your birthday
Decide if active employer coverage lets you delay Part B. Pull together your doctor list and current prescriptions now.
- 2
3 months before
File for Part A and Part B through Social Security unless you already draw benefits, in which case your card ships automatically.
- 3
2 months before
Pick a lane: Original Medicare paired with Medigap and a Part D plan, or a Medicare Advantage plan. Check networks and drug tiers before you commit.
- 4
1 month before
Get the paperwork in so your ID cards land before your start date, and confirm your current coverage won't lapse in between.
- 5
After your birthday
Your six-month Medigap open enrollment clock starts the day Part B begins. It's the only window where a health condition can't cost you the plan.
Still working?
An employer plan changes the timeline
If you or your spouse are still working and the group plan covers 20 or more employees, you can typically put off Part B without a penalty and stay on that coverage. Most people still sign up for Part A since it usually carries no premium, unless you're funding an HSA, because any part of Medicare stops HSA contributions cold.
Once the job ends, an eight-month Special Enrollment Period opens for Part B, and COBRA or a retiree plan won't stretch it further. Getting this wrong is one of the costliest mistakes we see, so run the numbers with our Part B penalty calculator first.
Smaller San Antonio employers work differently
If the company has fewer than 20 employees, Medicare typically becomes primary at 65, and putting off Part B can leave real gaps in your coverage. Check with HR before you decide anything.
Choosing
Two paths once you turn 65
- Original Medicare plus Medigap plus Part D: any doctor who accepts Medicare, a higher monthly bill, costs you can predict
- Medicare Advantage: lower premium, a defined network, copays per visit, and extras like dental or vision
- Either path still requires your monthly Part B premium
- An independent San Antonio agent can lay both out side by side at no charge
Compare Medigap against Medicare Advantage, or start with the Medicare checklist.
Let's confirm your exact enrollment dates
Our team will pin down your dates, tell you if your current job lets you delay Part B safely, and walk through both coverage paths. No cost, no pressure to sign anything.
